Consultations
Initial Consultation
A referral letter is required for your initial consultation. Referrals from your GP are valid for twelve months, and for three months from a Specialist.
Please arrive 15 minutes earlier than your appointment time to fill out relevant forms.
For your first appointment, please remember to bring:
- Current relevant medical imaging, hearing test or other investigations that may be pertinent
- A list of your current medications
- Current referral, Medicare or DVA card and private health insurance details
- Your appointment letter
- Completed patient details and privacy policy forms. These forms can be downloaded here or filled out online here.
Pre-Consultation
In order to build a comprehensive medical profile you may be asked to complete various questionnaires sent to you either via email or by letter.
Subsequent Consultations
Your surgeon may need to see you again following your initial appointment. Your surgeon may request test results, investigations and ongoing monitoring of your condition. Subsequent consultations are normally 15 minutes.
