IMPORTANT NOTE: 

We require all persons attending our rooms to complete and submit a Screening Checklist prior to arrival at our rooms.

Each person attending the appointment, including any accompanying family member, will need to complete the questions.

If you have answered ‘YES’ to any of the above questions we will need to discuss this via phone before your appointment, and additional precautions may be required.
